Our trained crisis intervention team is available 24/7 to provide immediate support and stabilization in the comfort of your own home or community. We offer rapid response to mental health crises, on-site assessment and intervention, and connection to ongoing support and resources, including our eviction prevention program and mental health therapy.
We offer comprehensive, individualized care for youth and families facing complex challenges, including access to our eviction prevention program. Our team works collaboratively with you to create a personalized plan that addresses your unique needs, incorporating mental health therapy and building on your strengths. We utilize a team-based, family-driven approach, ensuring coordination of services across multiple systems while focusing on building natural support networks, including mobile crisis services.
The eviction prevention program and rehousing initiative assist individuals on their journey from homelessness to permanent housing placement. Connecting Thru Housing collaborates with each client to address the issues that led to homelessness, whenever possible. Low-income individuals and families can access support services, including temporary financial aid for their housing needs, as well as mental health therapy and mobile crisis services to ensure comprehensive support.
Services are available to families that have been evicted, as well as those currently homeless and residing in local shelters. Regional agencies combine their resources in an effort to end homelessness in the community.
Our Mental Health Therapy Department is dedicated to providing professional, trauma-informed care that supports the emotional and psychological well-being of individuals facing housing instability, particularly through our eviction prevention program. Our licensed clinicians offer personalized interventions to address various mental health needs such as anxiety, depression, trauma, and substance use recovery. By utilizing evidence-based practices and a person-centered approach, we focus on building resilience, promoting healing, and empowering clients to achieve lasting stability. Additionally, our mobile crisis services ensure that we create a safe, inclusive, and respectful space where every client is treated with dignity and compassion.
Connecting Thru Housing is a Peer Support Provider who helps people navigate mental health and/or substance use challenges using a recovery-focused, trauma-informed approach. Peer support is different from therapy—we bring trained support and lived-experience insight to help you feel understood, build practical coping skills, and take steady steps toward the life you want.
How we can help
The Transitional Youth Services Program is a home and community-based intervention that supports transition-age members (ages 16 up to age 21) with behavioral health diagnoses of mental health and/or substance use disorders in reestablishing the knowledge and skills necessary to live independently. Transitional Youth Services staff assist and support the member in identifying goals and addressing barriers to independence. This process considers all systems affecting the member, including family, school/work, peers, individual needs, and the community. All services are delivered in the member’s natural environment in order to increase the likelihood of sustaining the progress made during the intervention. The ultimate aim of the program is to give members the skills and resources to resolve and prevent future problems in areas of housing, employment, parenting, involvement with court or social services.
